Free Health Screenings for State Health Plan Subscribers

The South Carolina Public Employee Benefit Authority is offering comprehensive health screenings at USC Upstate Health Services now through December 16, 2016. This screening, a $300 value in many healthcare settings, is FREE in 2016 for employees, retirees, and their covered spouses whose primary insurance coverage is the State Health Plan (Standard or Savings Plan).  You are only eligible to participate once per calendar year.

To register for this FREE screening, please call Health Services at 503-5191 for an appointment and complete and bring with you these two forms: HEALTH_HISTORY_FORM–REVISED-3-2016 and Wellness Screening Notice and Consent.

If you participated in the December 2015 screening, then you will likely have a health history form on file and don’t need to fill out another. Appointments will begin at 8:30 a.m., and appointment times are assigned on a first come, first choice basis.  You must be fasting, nothing to eat or drink for a minimum of 10 hours, prior to your appointment. You may drink water to take your morning medicines. You will be reminded of your appointment date by text if you would like, by providing your cell phone number and carrier information.

Covered lab tests are a blood lipid profile (Lipid panel), blood chemistry profile (CMP) and hemogram (CBC). A copy of your lab results will be provided. Additional lab testing can be performed at the same visit with an order from a physician with a diagnosis code for why they are requesting the test. This would be billed to Blue Cross Blue Shield through Quest Diagnostics. However, you may incur additional charges if insurance does not cover the requested lab testing. Bring your insurance card to your appointment.
